About the Role
Active Care Group is recruiting a Children's Support Worker to join an Active Assistance care team supporting a four-year-old boy with complex health needs. This contract role is listed in Wrexham, Wales, with care delivered in the child's family home in Flint following his return from hospital. Initially, you will need to work shifts in hospital in Rhyl, helping provide continuity of support as he moves home.
Key Responsibilities
- Provide attentive, individual support to a young child with complex health needs.
- Follow agreed care plans and guidance to support his safety, comfort and wellbeing.
- Work initial shifts in hospital in Rhyl before supporting him at home in Flint.
- Work sensitively alongside his family and the wider care team.
- Observe and communicate changes in his needs to the appropriate team members.
- Support age-appropriate interaction and daily routines in line with his needs and abilities.
What You'll Need
- A caring, patient approach and a genuine interest in supporting children.
- A willingness to support a child with complex health needs and follow individual care guidance.
- Good communication skills and the ability to work respectfully within a family home.
- A reliable approach to shift work and working as part of a care team.
- The ability to work in Rhyl initially and in Flint once the child returns home.
